3 Steps to CEM Membership

Step 1.   Apply your school for CEM Membership

Option A: Online (RECOMMENDED)

  1. Provide your email address. You will be sent an email to confirm your email address.
  2. Complete your name and address details.
  3. Add proof of your name and age - Documents you need
  4. Add proof of your residential address in London - Documents you need
  5. Add a colour photograph of yourself - Photo requirements
  6. Agree to the terms and conditions
  7. Take a note of your application reference number so that you can track the progress of your Freedom Pass.

​If you are not able to add proof of your identity, address or photograph you can skip this section and instead return your documents by post. You will need to print a verification page to return in the post along with the required information

If you need help applying online, assistance and computers may be available at some local libraries and civic centres, however, please check first by clicking on your relevant borough at the bottom of this page.

Option B: By post

 

You can either download and print an application form or collect a form from your local borough. To find out where you can collect an application form from please click on your relevant borough at the bottom of this page.

  1. Complete and sign the application form
  2. Enclose a passport-sized colour photograph - Photo requirements
  3. Enclose proof of your identity and proof of your address - Documents you need
  4. Return in the post to:

Please make sure you put enough postage on the envelope to ensure your application is delivered.

  1. Provide your valid email address. 
  2. Click the link in your email to confirm your email address.
  3. Complete your online application. A confirmation message will appear on the screen. 
  4. Check your email. Acknowledgment Receipt is sent as downloadable pdf copy. Take note of your Acknowledgment Receipt Number; you will use it for the next step. No need to print, fax or mail.

  1. Download or print application form.
  2. Write all the needed information, and then sign the form.
  3. Make a photocopy of your completed form.
  4. Send the original copy to CEM through post mail. See sidebar for contact details.

 

Step 2.   Pay the membership fee

Pay the annual membership fee according to your selected option to pay (e.g. Bank Deposit). Send your proof of payment or deposit slip with the following details:

2.1.    

Acknowledgment Receipt Number. Copy the numbers on the upper left corner of your Acknowledgment Receipt.
2.2.    School Name. Write the complete name of your school.

Note: Write only the complete school name if your application was submitted through fax or mail.

 

Step 3.   Use the CEM Tests

Send a request to administer CEM tests in your school by email mmo [at] cem-inc.org.ph (subject: Request%20for%20CEM%20test%20administration) , or call our test administration coordinator at (02) 8804-2145 or 8895-8236.

If your school is located outside Metro Manila, please contact nearest CEM Center Coordinator in your area.
